Bella Natural Health - Fee Schedule

(Fee for Service Patients)

 

New Patients

 

Well woman visit:

Full history and physical, blood work and/or saliva kit and discussion of current health plan.  Leave with a treatment plan.

One half hour/$175.00

 

Homeopathy/Bioidentical hormones:

Full evaluation including history and physical.  Review of laboratory results, saliva kit, or prescription for blood work.  Treatment plan, prescription for bioidentical hormones, and/or homeopathic remedy.  Nutritional consult and exercise plan included. 

One half hour/$ 245.00

 

 

Established Patients


Well Woman visit:  

Friends of Bella Natural Health for annual exam with Pap smear and unrushed girl talk. 

One half hour/$125.00

 

Problem visit:  

Gynecological concerns requiring an exam.  

One half hour/$105.00

 

Laboratory review and hormone adjustment: 

Homeopathy or bioidentical hormones.

One half hour/$90.00

 

 

Telephone Consultations

 

New- Homeopathy / Bioidentical hormones:

Phone consultation with appointment time at beginning or end of the business day.  Laboratory blood work prescriptions and saliva kits are sent to the home with appropriate coding for insurance reimbursement.  Appointment time is one hour and is prepaid through the website at www.bellanaturalhealth.info  Fee: $245.00

 

Established- Follow up: 

History and conversation on symptoms.  Review of laboratory results, treatment plans. 

One half hour/$125.00

 

Telephone Messages

We always have your health and wellbeing at heart and want to answer any questions you have.  Call-in hours for questions are 12:00 p.m. to 12:30 p.m. and 5:30 p.m. to 6:00 p.m. on business days (based on Dr. Jones availability). 

 

Phone calls are limited to five minutes, with the  option for a prolonged telephone evaluation with an appointed time  for a fee of $25.00 for 15 minutes.

 

Getting Started

Payments may be made prior to your appointment at our website by credit/debit card, or PayPal.  Payments made on site may be made by cash, credit/debit card, or check.  Return check fee is $25.00. 

 

You are welcome to visit our website for paper work needed for your visit.  Please arrive 10 minutes prior to the visit if possible so that we may accommodate you on time.

 

A cancellation fee of $25.00 will be applied if the office is not called 24 hours prior to visit.  Our office may be reached additionally via email on our website at www.bellanaturalhealth.info.
